Maura Holmes Earns Certification as a Readmissions Professional

Maura Holmes, Director of Marketing at Family First Home Companions, recently earned her certification as a readmissions professional through the National Readmissions Prevention Home-Care-in-Long-Island-NYCollaborative, an organization dedicated to best practices in care transitions and readmission prevention for hospitals, skilled nursing facilities, and other post-acute providers.  This certification has given Maura the knowledge and skills to create hospital readmissions programs and to provide consultations to discharge planners in hospitals and skilled nursing facilities on how to prevent avoidable hospital readmissions.

Nearly twenty percent of Medicare patients discharged from hospitals are readmitted within thirty days of the discharge.  This costs the healthcare system multiples of billions of dollars as well as creating issues for patients that are at risk for having medical complications after a hospital discharge due to non-compliance with discharge instructions, not making it to their follow up doctor appointments in the community, and not taking their medications appropriately.

Maura’s knowledge and training in readmissions will allow her to create programs for Family First Home Companions that help to prevent their clients from returning to the hospital as well as working closely with local hospitals and skilled nursing facilities to ensure that patients transition smoothly from the facility to their home with the assistance of a home care service provider.
